• Attach payment receipt of $100. If you request the salon, then you also have to pay an additional $400 for yearly inspections.
• Don’t forget the proof of training attendance.

Instead of appearing in person, you may send the package of documents by mail. The processing period might last 2-3 weeks.

Renewal
